Apply to National Health Insurance service staff at service window of the township office.

Instruction for Name of Forms or Documents

1. The original and copy of veteran certificate or certificate of a veteran’s widow, one of each（return the original after being verified）. 

2. The original and copy of ID card and household certificate, one of each (return the original after being verified).

3. Transfer out application form.

4. Descendants that are at least 25 years old should attach related identification documents.

5. Seal of insured.

6. ID card and seal of agent.
