Apply to National Health Insurance service staff at service window of the township office.

Required Document

Instruction for Name of Forms or Documents

1. The original and copy of veteran certificate or certificate of a veteran’s widow, one of each（return the original after being verified）.

2. The original and copy of ID card, one of each (return the original after being verified).

3. One household registration transcript.

4. Return entry and exit documents.

5. Descendants that are at least 25 years old should attach related identification documents.

6. Seal of the insured.

7. Identification document of foreign spouses. (for example: ARC).

8. ID card and seal of agent.
