1. One application form for temple registration

2. Five temple registration forms

3. Declaration form for temple seal, certificate (affidavit), three of each.

4. Two copies of the front and back covers of ID card of the person in charge.

5. Letter of consent for the land use.

6. One address plate certification.

7. Land registration transcript and cadastral map transcription, one of each. 

8. Photo of temple (1 or 2 photos).
